MDS Coordinator - RN or LVN
The Buckingham is seeking a detail-oriented and knowledgeable MDS Coordinator, either with an RN (Registered Nurse) or LVN (Licensed Vocational Nurse) qualification, to join our skilled nursing facility team. The MDS Coordinator is pivotal in ensuring the accuracy of resident care plans and maintaining compliance with federal and state regulations. This role involves conducting assessments, coordinating interdisciplinary team participation, and fostering an environment of quality care for all residents.
Key Responsibilities:
- Coordinate and manage the Minimum Data Set (MDS) process to ensure compliance with all governmental regulations.
- Conduct resident assessments and interviews to gather accurate and comprehensive data required for the MDS.
- Collaborate with multidisciplinary teams to develop and update individualized resident care plans.
- Ensure timely completion and submission of all MDS assessments and supporting documentation.
- Regularly review and analyze data to identify trends and areas for care quality improvement.
- Participate in care plan meetings with residents and their families to discuss care strategies.
- Provide education and training to staff on MDS processes and the regulatory requirements.
- Act as a resource for staff, residents, and families regarding resident care and assessment procedures.
Qualifications:
- Current and valid Texas RN or LVN license.
- Previous experience in MDS coordination and long-term care settings preferred.
- Strong understanding of Medicare and other reimbursement guidelines.
- Detail-oriented with excellent organizational and time-management skills.
- Proficiency in using MDS software and electronic health records.
- Strong communication and interpersonal skills to effectively interact with staff, residents, and families.
- Ability to work collaboratively in a team environment while also independently managing responsibilities.
