MDS Coordinator -RN or LVN

We are seeking a detail-oriented and passionate Minimum Data Set (MDS) Coordinator to join our healthcare team. The MDS Coordinator will be responsible for managing the comprehensive assessment process for residents in accordance with federal and state regulations, ensuring the delivery of high-quality care, and enhancing the overall quality of life for our residents. The ideal candidate will have a strong understanding of the MDS process, excellent communication skills, and a commitment to patient-centered care.

**Key Responsibilities:**
- Conduct initial and ongoing assessments according to the Minimum Data Set (MDS) requirements and guidelines.
- Ensure accurate and timely completion of MDS assessments, including analysis of data and collaboration with interdisciplinary team members.
- Manage the MDS schedule, including care plan meetings, to ensure compliance and adherence to timelines.
- Monitor and track resident outcomes and quality measures, utilizing MDS data to support care planning and quality improvement initiatives.
- Collaborate with nurses, physicians, social workers, and other healthcare professionals to develop and implement individualized care plans based on assessment findings.
- Stay informed of regulatory updates and changes involving MDS requirements and coordinate staff training to maintain compliance.
- Assist in the preparation for audits and surveys related to MDS and quality measures.
- Provide support and training to nursing staff regarding MDS documentation best practices and assessment techniques.
- Analyze trends and patterns from MDS data to facilitate performance improvement projects within the facility.

**Qualifications:**
- Registered Nurse (RN), Licensed Practical Nurse (LPN), or equivalent healthcare professional with MDS certification strongly preferred.
- Previous experience in a long-term care setting is highly desirable.
- In-depth knowledge of the MDS process, regulations, and quality measures.
- Strong analytical skills with attention to detail and accuracy.
- Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
- Proficient in electronic health record (EHR) systems and MDS-related software.
- Ability to manage multiple priorities and meet deadlines in a fast-paced environment.
