POSITION SUMMARY

Assist with planning, organizing and developing the overall emotional and social needs of the residents are met/maintained on an individual basis. Provides general nursing care to residents in the retirement community and health center by performing the following duties listed below.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Connect residents and families to necessary resources and supports in the community.
  • Encourage residents to participate in healthy activities, example; Edgemere Masterpiece Living.
  • Perform Resident Psycho social assessments when needed
  • Assist /Educate residents and families to participate in decision making when there is a change in health condition.
  • Help residents/families adjust to admission to Plaza Health care services; possible role changes; exploring emotional/social responses to illness and treatment of a resident.
  • Educate residents and families on the different levels of health care available; entitlements, community resources and advanced directives.
  • When needed perform health screening and evaluation of new and current residents.
  • Follow up with incident reports, explore possible ways of preventing future such incidents.
  • Work closely with Home Health Providers and assisting residents with coordination of additional services within their apartments.
  • Enforce the Caregiver Handbook.
  • Ensure residents are receiving Quality care from caregivers, Follow up with complaints.
  • Monitor change of condition in residents and make recommendations on proper level of care.

QUALIFICATIONS

Educational Requirements and Experience:

  • One to three years related experience
  • Current Registered Nurse/ Licensed Vocational Nurse license in Texas preferred
